Valley Health Systems is seeking a passionate and dedicated Social Worker to join our Center for Senior Wellness. What You’ll Do

• Develop and lead our Dementia Support Program, including patient enrollment, caregiver education, behavior-management strategies, and resource coordination.
• Host a Monthly Dementia Support Group and a Monthly Parkinson’s Support Group, creating safe and supportive spaces for patients and caregivers.
• Provide individual and group caregiver support sessions to help families navigate challenges confidently.
• Conduct Advance Care Planning (ACP) sessions for patients and families.
• Address Social Determinants of Health, such as transportation barriers, caregiver shortages, guardianship concerns, and nursing home or assisted-living placement.
• Support patients and families through end-of-life decision-making and hospice referrals with empathy and clarity.
• Collaborate closely with an interdisciplinary team to ensure holistic, patient-centered care.

What You Bring

• Master’s Degree in Social Work.
• Active WV LGSW, LCSW, or LICSW.
• Supervision offered to LGSW to get fully licensed.
• Experience with geriatrics, dementia, Parkinson’s, or caregiver support preferred.
• Excellent communication skills, emotional intelligence, and a passion for serving seniors.
